Living Health Facility?

A Congregate Living Health Facility is a licensed residential healthcare setting designed for individuals who need ongoing medical care. Unlike large institutions, a CLHF provides care in a smaller, home-like environment, offering a more personalized and comfortable healing experience. 

Magnolia Congregate Living is a 6-bed, nurse practitioner-led facility designed for medically complex patients. Our high staff-to-patient ratio allows us to deliver hospital-level care with the attention, dignity, and comfort of home.

To provide compassionate, individualized care that promotes dignity, healing, and quality of life. Our team works as one - united in care, focused on healing - to support each resident’s medical recovery and overall wellness.

Who We Serve

​Congregate Health Living Facilities are ideal for individuals:

  • Requiring skilled nursing care 

    • ​IV therapy (antibiotics, TPN, hydration)

    • Tube feeding (PEG, G-tube, J-tube)

    • Chronic respiratory patients who are oxygen dependent, bipap or ventilator support 

  • Needing management for complex wound care

    • Stage III-IV pressure ulcers​

    • Wound vac

  • Having complex medical needs but prefer a more intimate setting than a hospital or nursinghome 

  • Deconditioned patients needing rehabilitation and medical oversight

  • Having multiple comorbidities requiring close monitoring

  • Failed SNF placements

  • Social and medical complexity

In-house Provider Visits

Regular visits by experienced medical providers to oversee care, manage chronic conditions, adjust treatment plans, and coordinate with specialists-ensuring continuity and high-quality outcomes. 

Diagnostic Testing

On-site labs and diagnostics:

  • Routine blood work

  • Point-of-care testing 

  • Mobile imaging services

Allowing for faster clinical decision-making.

Specialist Consultation & Care Coordination

Seamless coordination with specialists and consultations to ensure timely integrated care planning for complex cases.

Consultations may include, but not limited to:

  • Pulmonologist

  • Neurologist 

  • Cardiologist 

  • Wound specialist

  • Podiatrist

Wound Management

Comprehensive wound care services including treatment of:

  • Pressure injuries 

  • Surgical wounds

  • Diabetic ulcears

  • Complex non-healing wounds 

Utilizing evidence-based protocols to promote healing and prevent complications
